We want to sincerely thank you for selecting our practice for your healthcare needs. We look forward to meeting you and promise to provide you with the individualized care you properly deserve.

As a new patient at For Women Only, we want to make your check-in at our office as easy as possible. Please reference the checklist below prior to your first visit.

For your first visit:

• Check in 20 minutes prior to appointment time to finish up any last minute paperwork
• Bring completed New Patient Forms shown below
• Bring your current Insurance Card and Driver's License/Picture ID
• Bring any medical records that apply to the reason for your visit
• Bring insurance Co-Pay (usually shown on your insurance card)

Please address only the problems that you are scheduled for in order to properly allot the required time so we do not run too far behind.

Remember emergencies do happen and our providers do their best to see you in a timely fashion so if there is a wait, we will notify you to try to prevent time constraints.

Please complete the following forms. Pregnant patients should also fill out and submit the Genetic Screening Form.

For directions and a map please see the Office Information tab. As a reminder, we have 2 office locations, our main office in Overland Park, and our satellite office in Louisburg. We are located in Overland Park at Quivira Medical Plaza on the first floor, on the campus of Overland Park Regional Medical Center. The Louisburg office is located just East of Hwy 69 at the Louisburg exit next to the Price Chopper store.

Please give us at least 24 hour notice if you need to cancel your appointment. Unfortunately, we must stress our cancellation policy if you do not notify us in a timely fashion. Many patients wait months for their visit and if you cancel properly, we would be able to work these patients in more quickly.
